Privacy Notice for Employees, Job Applicants, and Contractors Resident in California

We do not sell your personal information.

We collect personal details like your name and address, other contact information, work history, and employment-related personal data. We also collect sensitive Personal Information like race/ethnicity because we are required to do so by law. We collect this information so that we can process your employment, and we will keep this information as long as the law requires.

 

SHC Services, Inc. dba Supplemental Health Care (“SHC,” “we,” “us,” or “our”) provides this Privacy Notice (“Notice”) for Employees, Job Applicants, and Contractors who are residents of the State of California. We refer to such California residents as “you” or “your” throughout this Notice. This Notice sets forth certain data privacy-related practices as required by the California Consumer Privacy Act of 2018, as amended (“CCPA”). Our standard Privacy Policy is available here.

For purposes of this Notice, “Personal Information” has the meaning as defined in the CCPA and includes information that is collected by SHC about you for employment-related purposes (or, in the case of a contractor, for purposes relating to the provision of services). Personal Information encompasses any information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, with you. This notice does not address our treatment of certain of your personal information that is excluded from CCPA’s scope, including any protected health information that is regulated by the Health Insurance Portability and Accountability Act (“HIPAA”).

“Job applicant” refers to any California resident who is a candidate for employment with SHC; “employee” refers to any California resident who is employed by SHC as a full-or part-time employee or temporary worker; and “contractor” means a California resident who provides services to SHC pursuant to a written contract. SHC employees, owners, directors, officers and contractors are referred to herein as “Workforce Members”.

 

Information We Collect

We may collect Personal Information from you in a variety of ways, including, but not limited to on our website, company intranet, your mobile device, a job applicant platform, through email, at physical locations, through the mail, and/or over the telephone. Personal Information does not include publicly available information obtained from government records, or information that has been de-identified or aggregated.
